Which is the best incision for a Breast Augmentation?

Wednesday, December 7th, 2011

Good question, and the answer is not simple. Surgeons are always looking for ways to improve patient outcomes. This has led to the development of remote incision placements to better hide scars associated with placing an implant for breast augmentation. The most popular and most used incisions are either placed near the fold under the breast or through the edge of the lower areola. These incisions provide for direct access to the breast allowing for the surgeon to have the greatest chance for success in creating the proper pocket for implant placement. Also, these incisions allow for easier control of potential bleeding. Larger sized implants are much easier placed through and incision under the breast or under the nipple.

The next popular approach for placement of breast implants is through the axilla (arm pit). The scar from this approach is nicely hidden so that there are no scars on the breast. Creation of a proper pocket for the implant is difficult because direct vision of the tissues is difficult. Although some surgeons have been successful with this approach, I find that it often leads to a pocket that is too lateral resulting in separated breasts and poor cleavage. To improve the chances for success, I recommend the assistance of video endoscopy to create the best pocket and to control possible bleeding.

A novelty that I believe is losing popularity is the umbilical approach. Few surgeons have found this to be successful because it is a completely blind procedure. Pocket placement and control of possible bleeding are extremely difficult. I believe that placement of silicone breast implants is impossible through this approach.

Ultimately, choose the approach that you believe will give the best result, but make sure your surgeon is experienced with this technique.

Hand Rejuvenation

Tuesday, November 22nd, 2011

The hand can show changes due to aging and sun overexposure just like other parts of the body. The concepts for treatment are similar – Restore lost volume, restore skin tightness and elasticity.

Lost volume can be addressed with injections of fat, Radiesse, or Juvederm/Restylane. The injections are well tolerated and offer great results. There is no comparative study to assess which technique is best but I believe all are good. I prefer to use Radiesse because it lasts long and there is not the added procedure of harvesting fat. Results will be complete after a few days of swelling.

Restoring the skin can be done with lasers. I recommend the Matrix Laser (Fractional CO2) or the Pearl Laser from Cutera. The newer tighter skin will be noticeable within 1 week. These lasers can also remove brown pigment spots. Lasers can also be used on the arms.

Dr. Gallegos firmly believes in preserving your beautiful features, while providing a naturally appealing enhancement. In fact, Dr. Galleogos will conduct a thorough consultation with you to determine the best approach to plastic surgery. He will also inform you about alternatives and adjuncts to surgery. In some cases, you may not need a facelift when you could be better served by another type of procedure. In other cases, you may desire an additional procedure in order to have balance and harmony for your face.

Facelift Alternatives: Facelift alternatives may be appropriate for people who are not candidates for facelift surgery. One of these alternatives may be injectables such as Botox to diminish crow’s feet, wrinkles between eye brows and forehead lines. Facial fillers may be other alternatives because fillers fill in deep facial creases from the nose to the mouth. Yet, injectables and fillers will not improve facial sagging. Facial sagging and select wrinkles can only be improved by a facelift. Laser therapy may be advised to help prevent facial sagging and sun damage related wrinkles.

Adjuncts to Facelift Treatment: At times, a forehead lift or eyelid surgery may be adjuncts to facelift treatment. These procedures may be staged in two different treatment sessions for an optimal outcome. Sometimes, injectables, facial fillers, chemical peels and/or laser therapy may be adjuncts to treatment for an ideal outcome.

A consultation is the only way to identify an appropriate treatment plan and to discuss alternatives or adjuncts to treatment with you. Board Certified Plastic Surgeon Dr. Gallegos invite you to schedule a consultation at his practice in Albuquerque.

New Mexico Breast Surgery

Tuesday, July 19th, 2011

New Mexico Breast Surgery is quite popular today among women and even some men. According to statistics provided by the American Society of Plastic Surgeons, over half a million breast surgeries were performed in 2010. These procedures included breast augmentation, lift, reduction and breast reconstruction. While over 18,000 males underwent male breast reduction or gynecomastia.

Whether it’s breast augmentation, breast lift or breast reduction, breast surgery tops the list of plastic surgery procedures in many parts of the world, not only in through out the US and in Albuquerque, New Mexico. This time of year is also quite popular. Let’s explore the popular choices and summertime considerations.

Breast Augmentation: For an increase in breast size, you’ll want to consult with a plastic surgeon as soon as possible to review all considerations, including: the choice between saline or silicone implants, implant profile type, placement location, incision location and the style you’d like to have to best fit your profile. With summer vacation time being apt to be acceptable for work time off, the roughly one week recovery time may not be a burden to fit in so you can enjoy immediate benefits. In fact, advancement in breast surgery has significantly reduced the risk of discomfort following surgery when compared to previous decades allowing many women to return to work within a week.

Breast Lift: Summer also makes a good time for this procedure with recovery time being between a week or two. The reward is often apparent within a few weeks, which allows for a great fit into stylish summertime dresses, casual attire and bathing suits. Refined techniques used in the breast lift may make this procedure very appealing with the option for smaller more inconspicuous incisions.

Breast Reduction: Breast reduction offers relief from discomfort for many women, in addition to refining the breasts to be more in proportion with the rest of the body. Breast reduction is one cosmetic surgery procedure that is sometimes covered by insurance companies. Insurance companies determine whether or not the procedure is medical necessary, and if so often cover the cost of the procedure.

Male breast reduction also known as gynecomastia is becoming more and more popular as many men have witnessed the instant results with little or no discomfort. Additionally the recovery time is minimal and men are able to return to daily activities almost immediately, depending on the technique used to perform the procedure. Gynceomastia is not covered by insurance, but all in all breast reduction rewards offer a more fulfilling lifestyle for many.

Albuquerque Rhinoplasty

Friday, July 8th, 2011

Albuquerque rhinoplasty may be the words that you typed into your Internet browser, but your desires are likely to be very specific for nose reshaping. People interested in nose surgery often wish to preserve a sense of original identity, while altering an imperfection. You may be interested in rhinoplasty to smooth out an irregular nose bridge, refine your nasal tip, improve difficult breathing, augment or reduce the size of your nostril or nose.

People in Albuquerque considering rhinoplasty tend to be most interested in a subtle refinement that can bring out balance and harmony to the face. Of course, the results may seem dramatic with a resolution to imperfections that may have influenced your social or professional life. Still, the results of nose surgery are designed to be complimentary and natural looking. Both youths who are fully grown and adult males and females seek out nose surgery in Albuquerque. In fact, it’s a popular procedure which may be due, in part, to the vast majority of people in Albuquerque being young adults, with a median age of 34. This is a time when many people become more serious about nose surgery, free from school obligations, limited finances, or are no longer an adolescent unable to make such a treatment decision.

In terms of the procedure, people interested in nose surgery may be candidates for the open or closed rhinoplasty approach- both of which are sensibly priced for most people today. The open approach focuses on incisions made outside the nose in inconspicuous locations. The closed approach involves incisions made inside of the nose. The procedure is generally performed on an outpatient basis. The recovery time period may be up to a week, at which time swelling subsides. Recovery aids offer comfort throughout this time period. The results are then evident and often rewarding in more ways than one.

If you are interested in rhinoplasty, please contact our office of plastic surgery in Albuquerque, New Mexico.

Facelift – Albuquerque You Owe it to Yourself

Thursday, September 2nd, 2010

It’s an unfortunate fact of life to begin not to like what we see when we look in the mirror. Eventually aging affects all of us. No one likes looking tired, wrinkled, or droopy, anywhere in the face or neck area. Facial sagging can make anyone look older than they really are and you may start to consider cosmetic surgery treatment to correct this. Noticeable jowls, creases around the mouth and nose, lines under the lower eyelids, or loose skin under the jaw and chin are all indicators that time is advancing on you.

The things that bother you most about the appearance of your face can be discussed with Dr. Miguel Gallegos at Hermosa Plastic Surgery in Albuquerque, New Mexico. In the past several years, facelift procedures have advanced greatly and for the better. This has resulted in more natural looking results for patients. Dr. Gallegos is a board certified plastic surgeon, and has extensive training and experience in considering facial aesthetics when performing a facelift. You will want this quality in your doctor to achieve attractive rejuvenation and youthful beauty to your appearance. Your attitude will certainly get a big boost, also.

As the facial skin ages, it loses it flexibility and thickness. The skin begins to sag and wrinkle. Subcutaneous layers of fat are lost, and the skin dries out. Blemishes from too much sun exposure or poor skin care can occur. Skin cancer or burns can also be a reason to seek a facelift to improve the appearance of the epidermis. Superficial reasons are not the only ones considered in contemplating a facelift. Psychological reasons are valuable also. Looking better helps all of us to feel younger and more motivated, just as looking worse often influences us to feel older and worn out.

To check and see if you are a good candidate for a facelift, talk with your doctor. The procedure requires short incisions behind hidden locations, like the ear or hairline area. Loose skin, tissue, or fat that is causing your face to droop is carefully removed. It is imperative that just the right amount of tissue is discarded for natural looking results. Recovery time can take several weeks, and pain medication can be given to manage discomfort. Other procedures can be performed such as rhinoplasty, to eliminate additional recovery time and give you the overall enhancement you desire. Isn’t it about time that you looked your best, inside and outside?

Turn back the clock by discreetly tightening your underlying facial muscles and skin. Arrange a free consultation today by contacting my office, Hermosa Plastic Surgery located in Albuquerque.

Rhinoplasty – Do You Know What It Is?

Friday, August 13th, 2010

‘Rhinos’ is Greek for nose, and ‘Plassein’, or plastic, also from the Greek, means ‘to shape’. Thus, rhinoplasty is formed as a word. This is a surgical procedure that is performed on the nose by a head and neck surgeon, a maxillofacial surgeon, or a plastic surgeon. Rhinoplasty is carried out to reshape a nose for cosmetic reasons or for health reasons. Breathing problems due to trauma or a birth defect can be treated with rhinoplasty. For example, it is not uncommon for a sports injury to the nose to develop into a breathing problem. This would be one health reason why rhinoplasty would be completed on a patient.

More commonly known as a ‘nose job’, a cosmetic surgery on the nose can result in a face that is more contoured and pleasing to the eye. Did you know that the first nose surgeries have been recorded in history as far back as Circa 500 B.C.? Sushruta Samhita, a physician who lived in Ancient India, is regarded as ‘the father of plastic surgery’. A text in which he describes nasal reconstruction dates at 500 B.C. The techniques which he used to carry out forehead flap rhinoplasty have continued to be used, relatively unchanged, up to today.

The general reason why many seek nose surgery is because they think it is too big. Some want an undesirable bump or tip removed. Asymmetrical noses also annoy some. At Hermosa Plastic Surgery in Albuquerque, New Mexico, Dr. Michael Gallegos will be happy to meet with you and discuss the procedure of rhinoplasty. First of all, you need to ask if you qualify as a candidate. Dr. Gallegos is also skilled at picturing the final outcome of your nose reshaping. This is very important, because you want all of your facial proportions to be in balance and harmonize together for an overall attractive finish. Only the best plastic surgeons can visualize the finished results.

Depending on the desired outcome, an open or closed rhinoplasty can be executed by Dr. Gallegos, a board certified plastic surgeon who also provides services to the county of Santa Fe . In an open rhinoplasty, a small incision into the tissue is made that separates the nostrils. This way the doctor can have access to the bone by lifting the nose tissue. This procedure would be used to reduce a bump. In a closed rhinoplasty, the doctor will work within the nostrils. No scar will be visible at all between the nostril openings. This method limits the doctor’s access, but is good for simple, subtle changes to the nose. A free consultation can be scheduled to see what procedure will work for you.

If you are interested in rhinoplasty, contact Dr. Gallegos’s office to schedule a consultation.
