Please fill out the form below with as much detail as possible. If you would like to send us pictures of the area of interest you may upload them. This form is HIPAA secure and images will only be seen by our staff. In most cases, it is best to have someone take these photos for you.
After reviewing your form, our office staff will reach out to you to schedule an online consultation. You will be emailed a secure link to connect with our team via a HIPAA-compliant video conferencing tool at a scheduled appointment time. There is a $50.00 fee for virtual consultations, and once cleared for Elective Surgery we are offering reduced pricing.
